  • Are you always able to stop using drugs when you want to?
  • Have you ever had blackouts or flashbacks because of drug use?
  • Do you ever feel bad or guilty about your drug use?
  • Does your spouse (or parents) ever complain about your involvement with drugs?
  • Have you neglected your family because of your drug use?
  • Have you engaged in illegal activities to obtain drugs?
  • Have you experienced withdrawal symptoms when you stopped taking drugs?
  • Have you had medical problems related to your drug use (e.g., memory loss, hepatitis, convulsions, bleeding)?

Scoring:

Each “YES” answer (except question 3) = 1 point.

For question 3, a “NO” = 1 point.

A higher score suggests a greater degree of drug-related problems.

What does your score mean?

  • 0: No problems reported.
  • 1–2: Low level of drug-related issues.
  • 3–5: Moderate level — professional consultation recommended.
  • 6–8: Substantial level — professional help strongly advised.
  • 9–10: Severe level — immediate professional intervention needed.
